Mission
ClearReport is an AI-powered incident reporting platform for California assisted living facilities. It digitizes LIC 624, LIC 624A, and SOC 341 regulatory forms, provides AI liability review, compliance deadline tracking, and multi-facility management — so your staff can file faster and your facility stays protected.
How It Works
From report creation to PDF export in four steps.
Staff Files Report
A care worker fills out a guided wizard form, selecting the correct report type and entering incident details.
AI Liability Review
The AI scans the report for legally risky language and suggests safer alternatives before submission.
Admin Reviews
A facility or org admin reviews, approves, or sends the report back with notes for correction.
PDF Export
Approved reports are exported as filled regulatory PDF forms ready for filing with agencies.

AI Liability Review
Automatically scans report descriptions for legally risky language: admissions of fault, speculation, emotional language, and vague wording. Suggests safer alternatives inline.
AI Chat Advisor
Describe a situation in plain language and the AI determines whether it is reportable, which form to use, and applicable deadlines and agencies, reviewing against California 22 CCR §87211 and your facility-specific criteria.
